Aging in the Know:

Aging in the Know is your gateway to health and aging resources on the web. Created by the American Geriatrics Society Foundation for Health in Aging (FHA), Aging in the Know offers up-to-date information for consumers on health and aging.

The American Academy of Clinical Toxicology (AACT) is a not-for-profit multi-disciplinary organization uniting scientists and clinicians in the advancement of research, education, prevention and treatment of diseases caused by chemicals, drugs and toxins. The AACT was established to:

• Promote the study of health effects of poisons on humans and animals.
• Unite into one group scientists and clinicians whose research, clinical, and academic experience focus on clinical toxicology.
• Foster a better understanding of the principles and practice of clinical toxicology.
• Encourage development of new therapies and treatment in clinical toxicology.
• Facilitate information exchange among individual members and organizations interested in clinical toxicology.

This site is for dialysis patients and others who want to learn about the quality of care at individual dialysis clinics. Among other things, you can learn how often patients treated at a facility have been hospitalized, report certain types of infections or are placed on the transplant list. The information is submitted by facilities and collected by contractors of the Centers for Medicaid and Medicare Services, the federal agency that oversees most dialysis care.

Network of Care was created with a California Department of Aging innovation grant. The project is part of a broad effort by our county to improve and better coordinate long-term care services locally.

The Alzheimer's Association operates a Helpline 24 hours a day, seven days a week, in 140 languages. Their staff is highly trained and knowledgeable about all aspects of Alzheimers disease. You can also call us for emotional support as often as you need. We know that living with Alzheimers can be overwhelming at times. Remember, we are here for you all day, every day.

The Coalition is a grass-roots organization made up of volunteers committed to raising public awareness of the issues at the end of life. They believe that knowing more about the options for end-of-life care, whether it be in a hospital, nursing facility or at home, will improve the quality of living, and better prepare the person for this important stage of life.

An integral part of the Coalition program is educating the community about the importance of advance care planning and completing an Advance Directive for Health Care. The Make Your Wishes Known Initiative is dedicated to this end.

The Santa Cruz County Health Services Agency staffs the Santa Cruz County Tobacco Education Coalition, an advocacy group that promotes a tobacco-free lifestyle and environment. The Coalition was formed in 1985 and is currently funded by Proposition 99, the sales tax on cigarettes. The Coalition consists of community organizations as well as concerned grandparents, ex-smokers, and people who have lost a loved one to smoking.
